RESPECT IN SPORT

If you will be a coach, assistant coach, trainer or manager then you must complete the Respect in Sport - Leader program, which is available here.
 


Vulnerable Sector Check

All volunteera with the PLMHA must have a Vulnerable Sector Check on file with the association and must redo the check every two years. Print this letter and get it signed by a member of the PLMHA executive.


NEW REQUIREMENT FROM HEO: Gender Identity Training

All team officials must take the following two free online courses on gender equality:

There are two courses which will educate team officials on this important topic - Understanding Discrimination based on Gender Identity and Gender Expression and A Guide to the Dressing Room Policy and Confidentiality Statement.
All coaches and trainers must take these two courses and coaches will be awarded 3 certification maintenance points for taking the courses. HEO recommends that all team officials and administrators take this training given its significant benefit. For team officials already assigned to a team, completion of these modules will be required by October 1, 2017.

Gender Equality Dressing Room Policy

If you will be coaching or managing in the PLMHA then you should read and be familiar with the Gender Equality Dressing Room Policy, which has been adopted by Hockey Eastern Ontario.
 


Trainer Qualifications

Click here to register for courses. You will need to select "Trainer" from the top menu, then "Register for a clinic"  and then either "Level 1" or "Level 2/3".


Player Medical Information Form

The player medical information form should be completed by each player/guardian at the beginning of the season.  The medical forms are maintained by the Team Trainer and must be accessible at every team function, this includes but is not limited to games, practices and tournaments.

Player Injury Report Form

An Injury Report Form must be completed in the following cases:
  • If a player or team official is injured during a game and misses more than one period.
  • If a player or team official is injured during a practice or other association-sanctioned function and requires medical attention.
  • If a player or team official reports an injury after the fact (e.g. player is injured during a game and continues play, however attends a doctor the next day for the injury)
