PLMHA YEAR END BANQUET/AWARDS CEREMONY 2023-24 Season
All nomination forms must be submitted by February 28, 2024
A number of awards are available and presented during the year end banquet/awards ceremony. Nominations are accepted from coaching staff and individuals. All nomination forms must be emailed to the
perhlanarkminorhockey@hotmail.com by
February 28th. The coach evaluations should be dropped off in the brown box at the Perth Arena (same one used for raffle tickets) or emailed to plmha.president@gmail.com.
A chart is available listing the special awards available by Division and Position.
Nomination for Adult Awards
Any one (adult or player) can submit a nomination for these awards. All nominations
must be signed.
Nominations are accepted for (1) Coach or Coaching Staff of the Year and (2) Volunteer(s) of the Year. Click

Player Special Awards
There are quite a few Special Awards for players (forwards, defense, and goaltenders) that are selected each hockey season. First the team officials and those that watch several teams over the season send in their nominations, then a committee reviews those nominations and selects the recipients. Click

Team Awards
These awards MUST be chosen by each team. There should be only one person chosen per award. If there really has to be a tie please attach a short write up explaining why the tie was decided. Only 1 tie per team please (max. 4 names). Click

TEAM OFFICIALS EVALUATION
Each player/family is asked to complete an evaluation form for the coaching staff at the end of every season. These forms may be submitted at the the year end awards ceremony. The forms need not be signed and should be dropped off in the brown box at the Perth Arena (the same one used for raffle tickets) or emailed to plmha.president@gmail.com. The evaluation forms provide a means for the Association Executive to reveiw the minor hockey program and identify areas for improvement. Please click
